Clinic@Home is HealtheMed's flagship service: a technology-enabled, high-touch care model that wraps daily engagement and clinical monitoring around each client. It combines three layers of dedicated support with an assistive technology ecosystem to keep clients stable, supported, and independent.

The technology is tailored to each client's needs and may include remote patient monitoring devices, medication management tools, an in-home display, tablet, or phone-based mobile app, telehealth visits, and a dedicated care team.

When hands-on support is needed, our team can also visit clients in person.
