HealtheMed partners with home and community-based service providers, residential support organizations, and community health partners to support waivered clients with complex needs and barriers to care.
We work alongside your existing services. Our role is to fill the gap between clinical visits with daily engagement, proactive monitoring, and early intervention that keeps clients stable at home and reduces avoidable crises.
HealtheMed complements waiver services and skilled nursing. We support long-term stability while existing services handle acute and episodic needs.
A streamlined two-code billing structure (S5165 + S5110/S5115) approved across all six waivers: CADI, DD, BI, EW, CAC, and AC.
HealtheMed serves waivered clients across 35+ Minnesota counties through both traditional waivers and CDCS, with dedicated territory representatives.
Case managers receive periodic updates on client progress. Care coordination is built into the service, not an afterthought.

HealtheMed is approved for service authorization across all six Medicaid waivers, available for clients on traditional waivers and CDCS.

Our clients range in age from 4 to 87. About 79% manage two or more chronic conditions, and roughly 20% are homebound. Common diagnoses include hypertension, depression, anxiety, PTSD, diabetes, obesity, insomnia, and asthma.
HealtheMed is designed to reach high-needs, low-engagement populations. We are often a strong match for clients with chronic diagnoses, functional or mobility limitations, recent ER visits, barriers to in-person care, or a desire to live independently.
Review whether a client on a waiver (traditional or CDCS) could benefit from additional in-home and assistive-technology support.
Submit via phone or at healthemed.com/referral with client consent.
HealtheMed conducts a consult with the referral to assess fit and suitability for services.
A request for services is submitted to the client's case manager for budget review and approval.
If approved, HealtheMed works alongside the existing care team to begin engagement. A clinical consultant visits the client's home for assessment and technology setup.
