For individuals, family members, or caregivers who want to refer themselves or a loved one for HealtheMed's in-home support services.

HealtheMed is often the most beneficial for clients who have two or more of the following:
- Two or more chronic conditions
- Limited mobility or functional limitations
- Frequent ER visits or hospitalizations in the last 12 months
- Desire to live independently in their community
- Need for ongoing support with health and well-being
- Barriers to accessing regular doctor or mental health visits
