Clinic@Home Impact Report: State of Minnesota

HealtheMed serves clients on any MN Medicaid waiver across the state by removing barriers to equitable access to care, including challenges with transportation, financial, physical and growing mental and behavioral health needs. Our innovative in-home system of care integrates high technology and high-touch services to more effectively support this at-risk population in their quest to remain independent while lowering their out-of-pocket costs.

This Impact Report demonstrates the efficacy and viability of a system of enhanced virtual access to care, including significant improvement in engagement, medication adherence, social and mental stability.
