The Clinic@Home Platform provides an option for individuals who support active participants to view the same information presented to the participant on the Clinic@Home TV or Mobile App. These individuals could be a family member, a Case Manager, or another trusted person who assists the active participant. With the approval of the active participant, they are granted access to the Clinic@Home Portal, where they can see:
- Clinic@Home Health Score (see below for more details)
- Vitals: readings from assigned RPM devices, such as a Blood Pressure Monitor or scale.
- Medication information: Medication list, medication schedule, and medication acknowledgement.
- Device Compliance: RPM device usage
- Education: videos assigned to the client and watch status
- Devices: assigned RPM devices
- TV App: version, timezone, and language settings.
Account Set-up
- The active participant (aka client or patient) needs to approve access.
- Name and email need to be provided to Clinic@Home staff for account creation.
Ongoing Account Access
Please go to https://portal.healthemed.tv/ to access participant information in the Clinic@Home Portal. An email can be entered there and a code will be emailed to log in.
Navigate to the Client List to find the profile of the Clinic@Home participant. In the Profile, you can see the Health Score that demonstrates interactions with the Clinic@Home platform as well as other information.

Clinic@Home Health Score
Health Score = (Vitals Score + Medication Adherence Score + Device Compliance Score + Education Score)/Number of Available Scores.
- Vitals Score: This score looks at the data collected from RPM Devices and compares how many results were in normal range compared to all results collected over the month. A high score means that the participant is recording vital signs in their normal range.
- Medication Adherence Score: This score compares how many times the participant acknowledged taking medication compared to how many times they were scheduled to take medication during the month. A high score means that the participant is taking medications as scheduled.
- Device Compliance Score (or My Device Data): This scores compares how many times the participant used RPM Devices to get readings compared to the goal to use them during the month. A high score means that the participant is taking device measurements as expected.
- Education Score (or My Learning): This score compares how many times the client watched videos compared to the education goal to watch them during the month. The same goals will apply to all participants. Current education goal is for the clients to watch any video 2 times within a month.
