At the 129th Annual MSSA Training Conference & Expo, Shannon Holley-Smith, BSN, RN spoke to a full room of waiver case managers about Clinic@Home, our in-home system of care that helps waivered clients overcome barriers to care access and become more independent, while helping case managers more effectively manage their large caseloads.
Shannon uses real-life case studies to demonstrate how Clinic@Home, which is covered 100% by the Minnesota Medicaid waiver program, incorporates hi-technology with hi-touch services to improve the lives of all types of waivered clients.
